Fachsprachprüfung (FSP) for doctors: rules by state

The Fachsprachprüfung tests the medical German you need for your Approbation, at C1 level. Your state's medical chamber (Ärztekammer) or authority runs it. You cannot choose a date on a public calendar. You apply for your licence first. In some states the authority registers you; in others you register with the chamber after the authority's receipt. The chamber assigns the date.

No public calendar. Chambers assign dates. We cannot alert you to seats. We alert you when the official pages change.

COMMON QUESTIONS

Fachsprachprüfung for doctors, explained

What is the Fachsprachprüfung (FSP)?

The medical German test for doctors trained abroad. You need it for your Approbation or a temporary licence (Berufserlaubnis). It checks German in a medical setting at C1 level. It does not test medical knowledge.

What happens in the FSP?

In Lower Saxony it has three parts of 20 minutes: a simulated talk with a patient, a written doctor's report on that patient, and a talk with a doctor on the panel about the case. Check your state guide for your chamber.

How much does the FSP cost?

It depends on the state. Examples from the official pages: Westphalia-Lippe 400 EUR, Rhineland-Palatinate (Mainz) 470 EUR, Bavaria 550 EUR, Lower Saxony 595 EUR, Hesse 650 EUR. The table above lists every state.

Can I book the FSP myself?

Not on a public calendar. You apply for your Approbation or Berufserlaubnis first. In Lower Saxony and Bavaria the authority registers you once your file is complete, and the chamber invites you to a date; in Lower Saxony you pay only after NiZzA asks you to. In Hesse and Thuringia you register with the chamber electronically after the authority's step. Your state guide has the details.

Can I retake the FSP?

In Lower Saxony you repeat the whole exam, and there is no limit on attempts. Other states set their own rules.

Does the PKT replace the FSP?

Only in some states. FIA says that for EU degrees the PKT is recognised in Hamburg and Hesse; with a non-EU degree you take the chamber's Fachsprachprüfung there. FIA says that in Saarland the PKT Medicine and Pharmacy counts for all degrees. For dentistry, ask first. Your licensing authority decides.

FSP or Kenntnisprüfung: what is the difference?

The FSP tests your medical German. The Kenntnisprüfung tests your medical knowledge. Until 31 October 2026 you take it when the authority finds substantial differences in your training; from 1 November 2026 it is the normal route for degrees from outside the EU, the EEA and Switzerland. A doctor with such a degree may need both.
